Health Office

Holy Ghost Prep is dedicated to meeting the health and safety needs of our students. Responsibilities include implementation of state mandated programs including immunization surveillance and student health screenings as well as students immediate health care needs, delivering first aid and supportive nursing care. The Bensalem School District assigns a certified school nurse to visit as well.

Health Requirements

Any form listed on this page may be printed and sent to HGP or scanned and emailed to nurse@holyghostprep.org

According to Pennsylvania State Law, all children are required to have the following immunizations before they may ENTER school: Click here to view the required immunizations.

All 11th grade students are required to have a completed examination by your health practitioner. Click here for 11th grade physical/immunization form. You’re encouraged to have this exam done over the summer. Please send this in at the beginning of the school year, attention NURSE.

Medication in School

Medication to be given on a regular or “as needed basis” requires a written authorization from your child’s health care provider every school year. This includes Epi-pens, insulin and inhalers, as well as oral medications such as Ibuprofen containing medicine like Advil or Motrin. Have your child’s health care provider complete the Medication Authorization Form, Severe Allergy Form or the Asthma Form.

Medication that needs to be given for treatment of an acute nature, such as oral antibiotics, requires a note from your health care provider stating the time of day and length of treatment. All medications must be sent to school in the original pharmacy labeled container. A Medication Authorization form must be completed per medication.

If a parent requests that their child receive other medication during the school day (over-the-counter or prescription), the school nurse must be provided with a Medication Permission Form. This includes physician instructions from a licensed prescriber and parent/guardian and physician signatures. Other guidelines which must be followed for school administration of medication include:

  • Parent or guardian must deliver medication to school.
  • Medication must be in the original container with the students name, dose, frequency and time of administration.
  • If a parent is unable to obtain a physicians order, he or she may come to school and administer the medication to their child.
  • All medication orders must be renewed at the beginning of each academic year.
  • Medication that is not picked up at the end of the school year will be discarded.

Sudden Illness or Hospitalization

Prior to a student returning to school after treatment for pneumonia, mononucleosis or any hospitalization, the parent should inform the school nurse of the specific illness and provide a doctor's note on the first morning returning to school.

Pennsylvania state law mandates specific health screenings during the academic year. All grades will have growth and vision screenings and 11th grade will have hearing screenings as well.

The Nurse's Office is located on the first floor near the Chapel, and is open daily.
